Find Perio Patients Overdue for SRP

SQL Queries3 min read4/8/2026

If you are struggling to identify which of your periodontal patients have fallen through the cracks and are overdue for their scaling and root planing (SRP) or perio maintenance, this report is for you. This open dental SRP overdue query helps your hygiene coordinator quickly generate a list of active patients who have a history of perio procedures but no recent completion date for those specific codes.

The Query

Copy and paste the following code into your User Query window. This query looks for patients with a status of "Patient" who have had specific SRP codes (D4341 or D4342) in the past, but have not had a perio-related procedure in the last 12 months.

SET @MonthsOverdue = 12;
SET @StartDate = DATE_SUB(CURDATE(), INTERVAL @MonthsOverdue MONTH);

SELECT
    p.PatNum,
    p.LName,
    p.FName,
    p.HmPhone,
    p.WirelessPhone,
    MAX(pl.ProcDate) AS LastPerioProcDate
FROM patient p
INNER JOIN procedurelog pl ON p.PatNum = pl.PatNum
INNER JOIN procedurecode pc ON pl.CodeNum = pc.CodeNum
WHERE p.PatStatus = 0
AND pc.ProcCode IN ('D4341', 'D4342', 'D4910')
AND pl.ProcStatus = 2
GROUP BY p.PatNum
HAVING MAX(pl.ProcDate) < @StartDate
ORDER BY LastPerioProcDate ASC;

How to Run This Query

  1. In Open Dental, go to Reports in the Main Menu.
  2. Click User Query.
  3. Paste the query provided above into the large text box.
  4. Click Submit Query.
  5. The results will display in the grid below.

Understanding the Results

How to Customize

You can easily adjust the sensitivity of this report to match your practice's recall protocols.

Variations

If you want to narrow your focus, you can modify the pc.ProcCode line. For example, if you only want to see patients who have had Scaling and Root Planing (D4341/D4342) but have never had a Perio Maintenance (D4910) visit, you can remove 'D4910' from the list:

AND pc.ProcCode IN ('D4341', 'D4342')
